Preparing for Your Visit
Please arrive to your appointment 15 minutes ahead of time if you are a first-time patient, as there will be some intake paperwork to complete. Please bring your health insurance member card as well as a form of payment to cover your co-pay.

A Few Reminders
- Your co-payment is due upon receipt of services
- If you cannot attend your appointment, please give us 24-hours advance notice to avoid cancellation fees.
